This page contains information for both new and returning student-athletes regarding medical screens and clearances. ALL student-athletes must undergo an athletic medical screen and clearance performed by the CSUSM Team Physicians and Staff.

• In order to be eligible for medical screen and clearance all paperwork must be completed entirely and submitted on JumpForward. Due dates and medical screening dates are organized by sport. Medical clearances and screenings will not be scheduled until all paperwork is completed and submitted

• The following must be completed or uploaded and submitted on JumpFoward:
  
  
  1. Pre-Participatory Physical Screening Form
  2. Health Insurance & Emergency Information Form
  3. Attach a copy or photo of front & back of health insurance card
  4. Medical History Questionnaire Form (4 pages)
  5. Medical Clearance Form Signature Page
  6. Sickle Cell Trait Testing Form (incoming athlete ONLY)
  7. Attach a copy or photo of proof of Sickle Cell Trait testing (incoming athlete ONLY)
  8. ADHD Medication Exemption Form (if diagnosed with ADHD)
